Information you provide

The form asks for your contact details, address, age, coverage preferences, and general selections about existing insurance, tobacco use, medical treatment, and beneficiary relationships. Please do not submit medical records or diagnoses. Your answers remain in page memory until you select Send my request; the website does not save them in browser storage.

How your request is handled

When you submit, your answers and consent choices are sent through this website’s server to GoHighLevel, which supports contact management and scheduling. Abel V. uses them to respond to your request and discuss coverage options. Vercel hosts the site; hosting and CRM providers may process technical and request information to operate these services.

Your text-message choice

Automated texts are optional. You can submit your request and schedule a call without choosing texts. If you opt in, the site records the consent wording, version, time, and submission reference alongside your request. Texts concern your coverage request, booking link, and appointment scheduling and reminders. Reply STOP to opt out or HELP for help.

Sharing and protection

Mobile numbers and SMS opt-in information are not shared with third parties or affiliates for their own marketing or promotional purposes. Service providers may process information on Abel’s behalf to deliver the requested services. Information may also be disclosed when required by law. Please avoid submitting information beyond what the form requests.
